Application Checklist

You will need to provide copies of the following documents that pertain to your situation. Additional information may be requested as your application is in process. To expedite the on-line process you may want to gather the following documentation before you begin.

• Copy of your most recent pay stubs covering one complete month.
• Last two years W-2 statements or 1099’s if self-employed.
• Copy of most recent mortgage and home equity statements (if applicable).
• Copy of purchase contract (if applicable)
• Copy of statements for assets, such as checking, savings and 401K statements.
• If you receive alimony or child support, provide a copy of the divorce decree
  and evidence or receipt of income for 12 months.
• If you’ve had a bankruptcy within the last seven years, provide a copy of your
  bankruptcy discharge papers and a copy of the petition with all schedules.
